Direct Loans
Learn more about the federal loans included in your financial aid package that can ease the financial burden of earning your education.
- Asset Information Form - Complete and return the Asset Information Form.
- Previous Bachelor's Degree - You indicated on your financial aid application that you have already received a Bachelor's Degree. Submit a written statement verifying whether or not you have previously received a bachelor's degree.
- Student's Birth Certificate - Submit a copy of your birth certificate.
- Business Value and Debt - Complete and return the Value and Debt form.
- Child Support Paid Verification Form - Complete and return the Child Support Paid Verification Form
- Child Support Received - Submit documentation verifying the total child support you (or your parent, if you are dependent) received.
- Citizenship Document - Submit documentation that you are a U.S. citizen or an eligible non-citizen. Typical documents include a passport or a copy of both sides of your green card.
- Clarify Your Filing Status - Clarification of you or your parent's tax filing is needed.
- DD214 Form - Submit a copy of your DD-214 form to verify your Veteran status.
- Verify Disability Compensation - Submit documentation of the total amount of disability benefits you received.
- Drug Conviction-Ineligible - You indicated on your financial aid application that you have a drug conviction that is affecting your eligibility for financial aid. Please contact the Financial Aid Office to discuss your circumstances
- Other Family Member in College - You indicated on your financial aid application that another family member(s) will be attending college in the current academic year. Please follow the instructions on the Enrollment Verification Form to verify this information.
- No Final HS Transcript - The Admissions Office has not yet received your final high school transcript. This is required before we can award financial aid.
- Household Size Verification - Complete and return the Household Size Verification form.
- Identity and Statement of Educational Purpose - Must be completed and signed at the Financial Aid Office in the presence of a Financial Aid Administrator. Please come to the office with a government issued photo ID to complete this form.
- Additional Financial Information - Complete and return the Title IV Additional Financial Information Form.
- Living Circumstances - Complete and return the Living Circumstances Verification form.
- Loan Default - We have received information that you have defaulted on a student loan. You are not eligible for any form of financial aid until you resolve this default. Please provide documentation from your lender when you have cleared your default.
- Verification of State Residence - Please contact the Financial Aid office as additional information is needed before your eligibility for state financial aid may be determined.
- Verification of Marital Status - Provide documentation of your marital status (or your parent's status if you are a dependent student) as of the date you filed your Free Application for Federal Student Aid.
- NSLDS Match Problem - Our records indicate that you are currently ineligible for federal student loans. If you are interested in borrowing a student loan, please contact the Financial Aid Office. If you are not interested in a loan, please make a notation on this letter and return it to the Financial Aid Office.
- Other Information - Additional information is required to complete your application. Please call the Financial Aid Office for more information.
- Orphan/Ward of the Court Document - Submit documentation regarding your status as an orphan or ward of the court. Documentation of ward of the court must show that your were under state custody until your 19th birthday.
- Grant Overpayment - Your Student Aid Report indicates that you owe a repayment of federal funds. You are not eligible for financial aid until these funds have been repaid. Submit documentation to the Financial Aid Office when you have satisfied this payment.
- Parent's Non-Tax Filer Form - Complete and return the Non-Tax Filer Statement.
- Parent's Signature Missing - Our records indicate that your parent did not sign your Free Application for Federal Student Aid (FAFSA). Please refer to your Student Aid Report (yellow forms), which was sent to you by the Department of Education, and make the appropriate corrections.
- Parents' Social Security Benefits - Submit documentation, such as the 1099 form(s) , of your parent's total social security benefits received.
- Parent's Income Verification - Once your parent files their tax return, please verify their income online by going back to your FAFSA tax return information and choosing the IRS Data Retrieval option. If your parent is not able to use the IRS Data Retrieval you must submit an IRS Tax Return Transcript.
- Parents' W-2 Forms - Submit copies of your parent's W-2 forms. Include all forms received and be sure to include the student's name and social security number on the form.
- Needs Admit to a Program - You are currently not admitted to an eligible degree or certificate program. To qualify for financial aid you must enroll in a program. Please contact the Admissions Office at 508.854.4262 or click on the link to apply for admission.
- Savings and Checking Statements - Complete and return the Savings Verification Form.
- Student Needs to Readmit - You are no longer an active student at Quinsigamond Community College. Please contact the Admissions Office at 508.854.4262 or complete a readmission application by clicking on the link. Once you are admitted to an eligible program we can continue processing your financial aid application.
- Selective Service Registration - Submit documentation that you have registered with selective service.
- SNAP - Complete and Return the Supplemental Nutritional Assistance Program Verification Form
- Federal Tax Schedule - Submit all schedules filed with your federal tax return (1040). Be sure to include the student's name and social security number on these forms.
- Social Security Card - Submit a copy of your social security card.
- Spouse's Income Verification - Once your spouse’s tax return has been filed with the IRS, submit a copy of your spouse’s IRS Tax Return Transcript. IRS Data Retrieval Process.
- Spouse's W-2 Forms - Submit copies of all of your spouse's W-2's.
- SS Benefit Form - Submit 1099 statements for all members of your family who received social security benefits.
- Student's Signature Missing - Our records indicate that you did not sign your Free Application for Federal Student Aid (FAFSA). Please make a correction at fafsa.ed.gov and sign your FAFSA using your PIN.
- St/Sp Non-Tax Filer Form - Complete the Non-Tax Filer Form to verify your total untaxed income received.
- Student's Income Verification - Once you filed your tax return, please verify your income online by going back to your FAFSA tax return information and choosing the IRS Data Retrieval option. If you are not able to use the IRS Data Retrieval you must submit an IRS Tax Return Transcript.
- Student W-2 Form - Submit copies of all W-2's you received.
- Real Estate Value and Debt - Complete and return the Real Estate Value/Debt Form.
- Veteran Benefit Form - Provide documentation of the amount of your monthly Veteran's Benefits.
- Workmen's Compensation - Submit documentation of the total amount of Workers' Compensation you received.
- Untaxed Income Verification Form - Complete and return the Untaxed Income Verification Form
- Private Loan Applicant Self-Certification Form - Compete and return the Private Loan Applicant Self-Certification Form.
